Just discovered this one:

A Soul in a Bottle (Subterranean Press, 2006), an illustrated short story issued by itself in hardback. Runs 82 pages, but the pages are smaller than normal and the font larger than normal. The illustrations are by J. K. Potter and are very good. And so is the story, especially for any bibliophile.
